What exactly does a care coordinator do?

Care Coordination Services provide families with a single point of contact for everything related to their child’s care and support. Our care coordinators build and maintain an integrated care plan, communicate with providers on your behalf, manage insurance authorizations, and coordinate scheduling. They also help families navigate government programs their child may qualify for, from Early Intervention to school-based services.

Care Coordination Services at Nexus are included as part of your child’s service package at no additional cost to families. When your child receives any Nexus therapy service, a dedicated care coordinator becomes part of the team from day one to help guide communication, scheduling, and ongoing support.

However, even families with a single service benefit from coordination, particularly for insurance management, referral support and planning ahead for future evaluations. Also, as your child’s needs grow, your coordinator already knows your family’s history and can act quickly, so you never have to start over.

es. In fact, our coordinators are experienced with the NYC Early Intervention system and the CPSE/CSE process. For example, they can guide you from the initial referral through evaluation scheduling, IFSP and IEP meetings, and eventually the transition to school-age services. As a result, you will never have to navigate that process alone.

Your coordinator will agree on a preferred communication method with your family at intake, whether phone, email or secure messaging. After that, you can expect proactive monthly check-in calls, same-day responses to urgent questions and written summaries following any significant meeting or authorization event.
