Care Coordination Services help families manage the many moving parts involved in supporting a child with complex developmental or medical needs. From coordinating with pediatricians, therapists, schools, insurance companies, and government programs, Nexus assigns each family a dedicated care coordinator who builds a complete understanding of the child’s needs and develops an integrated care plan.

In addition, our Care Coordination Services ensure active communication between every provider involved in your child’s care. Whether your child receives speech therapy, physical therapy, occupational therapy, ABA, early intervention services, or a combination of supports, our coordinators help align services, track progress, and make sure nothing falls through the cracks.

Care Coordination Services at Nexus manage every touchpoint across your child’s care network to ensure seamless communication and support. Our team assists with therapy scheduling and provider communication for Speech, PT, OT, ABA, and Special Instruction services. We also coordinate medical and specialist care with pediatricians, neurologists, developmental pediatricians, and hospital teams.

Care Coordination Services also include insurance verification and prior authorization management for commercial plans, Medicaid managed care, and Early Intervention billing. Our coordinators help families navigate the Early Intervention and CPSE/CSE process with IFSP and IEP support, meeting attendance, and transition planning. In addition, we connect families with community resources, financial assistance programs, adaptive equipment, and ongoing progress monitoring to adjust care plans as your child develops.

What exactly does a care coordinator do?

Care Coordination Services provide families with a single point of contact for everything related to their child’s care and support. Our care coordinators build and maintain an integrated care plan, communicate with providers on your behalf, manage insurance authorizations, and coordinate scheduling. They also help families navigate government programs their child may qualify for, from Early Intervention to school-based services.

Care Coordination Services at Nexus are included as part of your child’s service package at no additional cost to families. When your child receives any Nexus therapy service, a dedicated care coordinator becomes part of the team from day one to help guide communication, scheduling, and ongoing support.

However, even families with a single service benefit from coordination, particularly for insurance management, referral support and planning ahead for future evaluations. Also, as your child’s needs grow, your coordinator already knows your family’s history and can act quickly, so you never have to start over.
